Autografts

High dose Melphalan 200 mg/m2 is the standard conditioning for autografts in myeloma. If renal function is impaired, e.g. creatinine clearance <50 ml/min and/or reduced eGFR, discuss with the SCT Committee about possible dose reduction of melphalan.

Standard myeloablative conditioning

Standard allografts are not often performed. Cyclophosphamide/TBI could be considered for a standard allograft.

RIC SCT is not currently offered to patients with myeloma.
