
Contributing Factors for Problems
- Failure to wash hands before inserting the cannula.
- Inadequate skin antisepsis prior to insertion of cannula
- Placing non-sterile dressings and tape over insertion site.
- Leaving cannula in situ longer than the recommended time. (CDC recommendations: may remain in for 72-96 hours or until the first sign of phlebitis occurs).
- Failure to replace cannula that are placed under emergency conditions (e.g. by ambulance staff or emergency department).
Documentation of procedure:
- Date, using green label on dressing and in clinical notes. The completed green label should be placed on the side of the dressing but should not obscure the insertion site. The green label should not be placed on IV tubing.
