Key Points
A pancytopenia with a hypoplastic marrow is characteristic of aplastic anaemia but other conditions need to be considered.
- Is it inherited, e.g. Fanconi Anaemia?
- Could it be hypoplastic MDS or AML?
- Is it an early manifestation of ALL?
In established AA:
- Is there any relevant history of drugs or viral infections in the previous 6 to 12 months?
- Is there any evidence of spontaneous recovery? If possible, delay definitive treatment for 4 to 6 weeks to allow improvement to occur.
