
Discharges and Coding
Failure to observe the correct discharge procedure can lead to considerable inefficiency and cause inconvenience to the patient.
Upon discharge from hospital, patients under the care of Haematology should be told to ring 364 0384 (HOP) if they are unwell or febrile during normal clinic hours (which are 0800h – 1630h, Monday to Friday).
Outside these hours they should ring the Haematologist on call via the hospital switchboard.
Discharges
- Are you acquainted with the correct discharge procedure (outlined in the Green Book - Medical Registrars and House Officers Handbook)?
- Start preparing the Discharge Letter on Health Connect South as soon as possible, preferably when the patient is admitted. This will include a list of discharge medications and details of follow-up arrangements.
- Wherever possible, a patient who is to be discharged should be discharged before midday. This can be facilitated by making discharge arrangements in advance as above.
- Make sure the nursing staff and the ward clerk are fully aware that the patient is being discharged, since they will need to make any necessary referrals and follow-up appointments.
- Ensure that the discharge summary (if required) is completed promptly and that the medical record has been appropriately coded.
- Contact with the GP is facilitated by sending the Discharge Letter electronically via Health Connect South.
- Transplanted patients are followed up after discharge by the Transplant Registrar. Please ensure that a first follow-up has been arranged and that all the notes including in-patient notes are available.

Coding
This is done on the ward by the Coding Team. Make sure the underlying haematological diagnosis is clearly stated in the notes because this is used by the coders for accurate internal accounting and to obtain payments in the case of patients who come from out of district. Ensure that all diagnoses are recorded.
